Cortisone shots have been used for a long time for a lot of medical conditions. As a sport 's med doctor, I think that cortisone has just few indications in sport's medicine (bursitis, swollen articulations, rhumatologic conditions, etc). With time, cortisone affects the quality of cartilage and can cause a rupture of tendons. Cortisone should always be injected under ultrasound guidance. Cortisone gives a rapid relieve, but doesn't last long.
We are allowed to inject cortisone three times a year only because it can cause damage to structures like ligaments, tendons, muscles and articulations.
In sport's medicine, we now have other options to treat muskulo-squelettal problems. Just read below, about viscosuplementation, PRP and prolotherapy.
Viscosupplementation is an injection of products like Synvisc One, Durolane or Suplasyn in your articulations. Those products are made with hyaluronic acid. It has the same composition as your own synovial liquid in your articulations. Viscosupplementation is very good in getting rid of your pain and helping to nourrish your cartilage. It's a gel that will help to protect your articulation from degenerative arthrosis. Compared to cortisone, we can give as much injection as we need per year. On the other hand, viscosupplementation is more expensive, but most of insurance company will cover it.
Prolotherapy is an injection composed with 50% dextrose (sugar) and 1% xylo (anesthetic). The injection of sugar creates a local inflammation and stimulates an inflammatory response which is necessary for desirable healing process. Prolotherapy stimulates the construction and repair of ligaments and tendons. It would also increase their strength. The injections are given every 4 to 6 weeks. A maximum of six treatments is recommended.
